Juan Plascencia

Juan Plascencia is a Fresno State graduate with a Bachelor’s degree in Public Health. He is a community-focused professional with experience in outreach, patient support, and connecting individuals to essential health and social services. Juan has worked within a federally qualified health center serving multiple counties, supporting underserved populations by improving access to critical resources and helping individuals navigate complex healthcare systems.

As Healthcare Partnership Specialist, Juan will work from the Fresno office while supporting CARES offices across the region. He looks forward to connecting with providers and community partners to better understand their needs and improve the accessibility and effectiveness of the referral process — contributing to work that directly impacts underserved communities and expands access to care.

Outside of work, Juan enjoys traveling, exploring local restaurants and coffee shops, and supporting local soccer communities.
