CCAC is dedicated to improving health outcomes for underserved and high-risk populations across Central California. Through a combination of direct services, health education, and care coordination, our team works to address asthma and social determinants of health in an equitable way. Our culturally competent staff provide in-home visits, health assessments, and personalized education to help individuals better manage their conditions and navigate the healthcare system.
This division leads key programs such as CARES (Comprehensive Asthma Remediation and Education Services) and Enhanced Care Management (ECM), which offer targeted support to Medi-Cal beneficiaries with complex health and social needs. By integrating medical, social, and environmental care, we ensure a whole-person approach that not only improves individual well-being but also strengthens community health systems. With a focus on equity, collaboration, and prevention, CCAC’s Community Health division is transforming how care is delivered, one household, one patient at a time.
