Perla Mendoza
CARES Community Health Worker II
Perla is a Cum Laude graduate of Sacramento State University, where she received a Bachelor’s in Public Health with a concentration in Community Health Education.
As a Community Health Worker, Perla collaborates with health plans, healthcare providers, and other CCAC contracted organizations to implement CCAC’s CARES and other related health supportive programs and projects, which may include but are not limited to assessments of the home environment, care coordination, and asthma education.
Perla, a native of Kern County and the daughter of farmworkers, is committed to serving as a link between the community’s residents and health and social services by offering support, resources, and education on health that are appropriate for their cultural contexts. In her free time, she enjoys exercising, trying new restaurants, and spending time with her loved ones.
