Graciela Anaya, MPH
Director of Community Health
Graciela is an experienced public health professional with a deep commitment to empowering communities in the San Joaquin Valley. She holds a Bachelor’s Degree in Public Health from Fresno State and a Master’s Degree in Public Health from California State University, Northridge. With over six years of experience in health education and advocacy in clinic settings, including her work at federally qualified health centers, Graciela has developed a strong foundation in delivering health services to underserved populations.
As the Director of Community Health, Graciela oversees the implementation of the asthma program across thirteen counties in the San Joaquin Valley, from Kern to El Dorado. In this role, she serves as the primary liaison between Managed Care Plans, Community-Based Organizations, and other partner organizations to ensure the success of public health initiatives.
Graciela has been pivotal in the contracting of CCAC as a Community Supports Service and Enhanced Care Management provider under the California Advancing and Innovating Medi-Cal (CalAIM) initiative, expanding access to essential health services for underserved populations. A proud Tulare County native and daughter of farmworkers, she is passionate about empowering rural communities to take charge of their health through education and accessible resources.
